Exhibitor Reservation Form
Please type all information into the form below. When complete, select Print Form and either email or fax the completed form to the International Association for Food Protection. A $400 deposit per booth is required to reserve your space. Full payment is due after March 29, 2013. All correspondence concerning exhibits will be directed to your designated contact person at the address provided below. Three complimentary meeting registrations will be provided for each booth space reserved.

Exhibit space must be reserved with a deposit of $400 per booth to IAFP before March 29, 2013. Exhibitors may take a $100 discount off the applicable space rental fee if fee is paid in full by January 31, 2013. Balance is due March 29, 2013. After March 29, 2013, the entire booth rental fee must be submitted with the space reservation. Cancellation of reserved exhibit space must be submitted in writing to the Assistant Director. Cancellations before March 29, 2013 will receive a full refund. On notifications of cancellation after March 29, 2013, a $400 processing fee per booth space will be charged. After May 31, 2013 no refunds will be made by the Association unless the space is resold.
